Part-time RN
The Homewatch CareGivers of Greenville Nurse role will ensure that patients receive high-quality, personalized care in the comfort of their homes while supporting a wide range of health needs.
Key Responsibilities:
· Conduct thorough assessments of clients’ needs and develop personalized care plans.
· Educate patients and their families on managing illnesses, medications, and nutrition.
· Collaborate with multidisciplinary teams, including physicians, therapists, and social workers, to coordinate comprehensive care.
· Monitor patients' vital signs and report any changes to the care team.
· Conduct quality assurance visits to ensure compliance with care standards.
· Supervise and train caregivers, ensuring adherence to state regulations and quality care protocols.
· Maintain accurate and timely documentation of patient care activities and progress.
· Other duties as assigned.
Qualifications:
· Valid RN license in the state of SC. Without pending or board action.
· Minimum of 4 years nursing experience, including at least 2 year(s) in home care or a closely related field.
· Strong clinical assessment skills and knowledge of home care practices; experience in geriatrics preferred.
· Excellent communication and interpersonal skills to build rapport with patients and families.
· Ability to work independently and as part of a dynamic team.
· Proficiency in using electronic health records (EHR) and other relevant technology.
· CPR and First Aid certification required; CPR Instructor certification preferred.
· Knowledge of SC Home Health regulations is strongly preferred.
· Reliable transportation and willingness to travel in the Greenville area.
Physical Demands:
· Ability to stand, walk, and move throughout the day, including traveling to different client homes.
· Capability to lift, transfer, and support clients, including helping them get in and out of bed, wheelchairs, or other assistive devices, which may require lifting up to 40 lbs.
· Frequent bending, twisting, and reaching to provide care, perform assessments, and handle medical equipment or supplies.
· Using hands for tasks such as administering medications, using medical equipment, and performing procedures that require masterful coordination and dexterity.
· A keen ability to observe client details through close monitoring, accurately interpret medical records, and clearly communicate findings both with clients and team members.
· Must be able to wear required PPE (e.g., masks, gloves, gowns) for extended periods as needed for infection control.
· Ability to work in various environments, including client homes, which may involve different levels of accessibility, lighting, and temperature.
